Welcome to Chase Brexton Columbia Center
Your road to staying healthy starts with your Primary Care Provider. When you become a Healthy Howard Health Plan member, you will be assigned a primary care doctor from Chase Brexton Columbia Center on Knoll North Drive in Columbia. This doctor will provide your primary care. And remember, before you see any specialists you must first receive a referral from your primary care provider.

As a Healthy Howard Health Plan member, you’re guaranteed up to six (6) primary care visits every year without any additional fee. In addition to these six visits, women enrolled in the plan also receive one annual gynecological exam without an additional fee. You are not limited to six primary care visits with your doctor, however you will be charged for any additional visits.
Meeting with your Referral and Pharmacy Benefits Coordinator.
Your personal Healthy Howard Referral and Pharmacy Benefits Coordinator is also located at Chase Brexton. Each time you meet with your primary care doctor you’ll also meet with your Coordinator. During these visits, your Coordinator can:
- Provide information about health services available to you through the Healthy Howard Health Plan
- Help you find reduced prices for prescription drugs
- Help you make any follow-up appointments with specialists within our network (if your doctor makes a referral)
